They have a medical condition that requires treatment or rehabilitative services prescribed by a physician, and
A physical and/or mental impairment limits activities of daily living, but not so seriously that it requires 24-hour institutional care, and
There's a reasonable expectation that preventative services will maintain or improve their present level of functioning, and
Without CBAS services, there's a high chance of further decline and eventual institutionalization.

The entire program cost may be covered by Medi-Cal, a Managed Health Plan, VA, or LTC insurance. A sliding-scale fee applies to participants who aren't covered by Medi-Cal (private pay).
